GLP-1 Injection Sites: Where to Inject and How to Rotate

October 27, 2026

Educational only. Not medical advice. Talk to your doctor.

Short answer: The prescribing information (labels) for Ozempic® (semaglutide), Wegovy® (semaglutide) and Zepbound® (tirzepatide) list the same three subcutaneous injection areas: the abdomen (stomach area), the thigh, and the upper arm. The labels also tell you to use a different injection site each time. Your prescriber and the Instructions for Use that come with your pen decide the details for you.

Where can you inject a GLP-1 medicine?

Each label names the same three areas. These are the exact words from the labels, current on DailyMed as of October 27, 2026:

MedicineWhat the label says
Ozempic (semaglutide)“Inject OZEMPIC subcutaneously in the abdomen, thigh, or upper arm.”
Wegovy (semaglutide)“Inject WEGOVY subcutaneously in the abdomen, thigh, or upper arm.”
Zepbound (tirzepatide)“Inject ZEPBOUND subcutaneously in the abdomen, thigh, or another person should inject in the back of the upper arm.”

Zepbound’s wording matters: the upper arm is listed for when another person gives the injection. MedlinePlus, the NIH patient-information site, describes semaglutide the same way: “Inject semaglutide in your upper arm, thigh, or stomach area.”

How do you rotate injection sites?

Rotation means not using the exact same spot every time. The labels word it a little differently:

  • Wegovy and Zepbound: “Rotate injection sites with each dose.”
  • Ozempic: “Instruct patients to use a different injection site each week when injecting in the same body region.”
  • MedlinePlus (semaglutide): “Use a different site for each injection.”

In plain terms, you can move between the abdomen, thigh and upper arm, or stay in one area and move to a new spot within it. Your prescriber can tell you which approach fits you.

Does the injection site change the dose?

The Wegovy label says that “the time of day and the injection site can be changed without the need for a dosage modification.” Never change your dose on your own. Follow your prescribing information and your prescriber.

Can you use the same area as insulin?

The Ozempic label says it is acceptable to inject Ozempic and insulin in the same body region, but the injections should not be adjacent to each other. If you use both, ask your prescriber how to arrange them.

What are injection site reactions?

Injection site reactions are local effects where the needle went in. The Wegovy label lists pruritus (itching), erythema (redness), inflammation, induration (hardening) and irritation. In its weight-reduction trials, 1.4% of Wegovy-treated adults and 1% of adults on placebo had injection site reactions. The Zepbound label groups bruising, redness, itching, pain and rash under the same term and reports 8% at the 10 mg and 15 mg strengths versus 2% with placebo.

How can you keep track of where you injected?

Many people jot down the area after each shot, on paper or in a notes app, so the next one lands somewhere new. Weeko, a shot tracker coming to iPhone and Android, is being built around this kind of simple log. It is a logbook, not a source of medical guidance.

When to call your doctor

Call your prescriber if an injection site becomes very painful, swollen, hard or red, or if the reaction does not go away. For signs of a serious allergic reaction or the thyroid warning symptoms on the label (a lump or swelling in the neck, hoarseness, trouble swallowing or shortness of breath), MedlinePlus says to call your doctor immediately or get emergency medical treatment. Questions about technique, needles or storage belong with your prescriber or pharmacist.
